Please follow this link for a closer look at what it's like to work at Children's Wisconsin: https://www.instagram.com/lifeatcw/ Assists Chorus Community Health Plan (CCHP) members with complex and chronic conditions utilizing the case management process, coordinating the medical, behavioral and social needs for members. The Case Management Outreach Coordinator, via telephonic and in-person case management, will be provided collaborative oversight by a clinical team of nurses and social workers to coordinate care for members and refer to appropriate community resources. Education (Experience can be substituted for education) High School graduate or Certificate of General Educational Development (GED) or High School Equivalency Diploma (HSED). Experience (Education can be substituted for experience) Five years of experience in health promotion, health advocacy, health education, community outreach, social service or other related field.Bachelor's degree with two years of experience may be considered in lieu of five years of experience. Knowledge / Skills / Abilities Exhibits guiding behaviors that reflect Children's values and support our mission and vision.Effective written and oral communication skills.Strong customer service orientation.Demonstrated problem solving skills.Ability to work independently and with teams.Working knowledge of Microsoft Office.Comfortable working in a fast-paced and changing environment. Preferred Job Requirements Licensure, Registration and/or Certification Community Health Worker and/or Social Worker Certification Education Bachelor's degree Knowledge / Skills / Abilities Knowledge of the Medicaid population and benefits.Ability to navigate specific member record programs such as EPIC, Forward Portal, Guiding Care etc.Bi-lingualThis is a 100% remote position M-F regular day time hours.Children's Wisconsin is an equal opportunity / affirmative action employer.
