Acenda's expert team helps each individual, family and community move forward in their journey towards health and happiness.If You Want Flexibility and To Make A Difference In The Community, Join Acenda's Integrated Health Care Team As A Case Manager for Our Integrated Case Management Services.Key Responsibilities: * Engages consumers at the point of hospitalization and participates in the discharge planning process to assist with the transition to the community * Provides post-discharge follow-up to support the consumer with community integration * Conducts assessments within correctional facilities and attends court hearings to advocate for persons served.Requirements: * Minimum requirement of a Bachelors Degree in Human Services or Psychology. * At least 2 years of experience in mental services preferred * Professionalism and a commitment to excellence in care * Must have and maintain a valid driver's license with an acceptable driving record.
